Dear Patient:

We ask that you please arrive 15 minutes prior to your appointment so that you may REGISTER directly in the RADIOLOGY DEPARTMENT. When you enter the front entrance of the hospital, this will be the first door on your left.

We ask that you do not eat or drink after midnight the day before your examination. (You may continue your medications.)

*PLEASE BE ADVISED that it is the policy of Mount St. Mary's Hospital and a MANDATE OF THE FEDERAL GOVERNMENT THAT WE HAVE A WRITTEN ORDER ON FILE FROM YOUR DOCTOR PRIOR TO PERFORMING YOUR EXAM/TEST. IN THE EVENT WE DO NOT HAVE THIS ORDER, WE WILL BE FORCED TO RESCHEDULE YOUR TEST TO A LATER DATE.

WE REQUIRE YOUR PRESCRIPTION FOR THIS TEST TO ENSURE WE TAILOR THE EXAM TO ANSWER YOUR MEDICAL PROBLEMS. IF YOUR PHYSICIAN HAS NOT GIVEN ONE TO YOU, YOU MAY ASK THEM TO FAX IT TO US AT 298-2256.
