Dear Patient:

We ask the you please arrive at least 30 minutes prior to your appointment to register. Outpatient Admitting is located in the rear of the building and is open Monday through Friday at 7:30 a.m. If you appointment is in earlier in the morning or on the weekend, you must then report to Main Admitting located off the Lobby through the main entrance of the hospital.

IMPORTANT - You MUST have a written order/prescription from your physician or you test WILL NOT be performed.

You do not need to follow any special instructions for this examination. If you have any questions about your examination, please feel free to call us at 298-2134.

*Co-pays & Deductables will be collected at the time of service. We accept Master Charge, Visa and personal checks.

- VALUABLES -

The Hospital will not be responsible for lost or stolen property. All valuable items should be left at home.
