Dear Patient:

We ask that you please arrive at least 30 minutes prior to your appointment so that you may register in Outpatient Admitting.

You MUST have a prescription from your physician with you on the day of the test.

PREPARATIONS FOR TEST:

1. Preparations at Home

a. No daytime naps.
b. Eat normally. NO CAFFEINE after 3:00pm.
c. Shampoo hair well day of test.
d. No hair spray, gel, mousse, or conditioners.

2. NO ALCOHOLIC BEVERAGES THE DAY OF THE TEST.

3. Bring to the hospital:

a. 2 piece pajamas
b. personal toiletries
c. a list of medications (bring them if they are to be taken)

You will go to bed at 11:00pm (no later) and wake up at 6:00am. unless you need to get up earlier for work, school, etc. Your test will require approximately 7 hours.

On the day of your test, report to admitting with insurance information at 8:30pm. If COMMUNITY BLUE is your primary insurance, you must have a REFERRAL on the day of your test.
