Speech & Language Pathologist
Per Diem. Responsible for delivering resident care through the process of assessment, planning, implementation and evaluation, as appropriate, to all ages of the residents served, based on areas of clinical practice. This includes, but not limited to assessment and treatment of people with disorders related to speech, language, voice, fluency and cognitive communication. Will work with people who have oral motor problems that cause eating and swallowing difficulties. Residents may include those with mental and emotional disabilities, hearing impairment, oral deformities, brain injury, stroke or degenerative disorders. Will design individual treatment plans to help residents develop or recover communication skills and counsel residents, staff and families in dealing with these disabilities. Collaborates with the interdisciplinary team for the coordination of services, making recommendations for diet, feeding, communication, and cognition.
Will complete MDS documentation, Momentous Charting, and care planning, physician orders. Will attend sub acute team meetings and team care meetings as needed. Will report to the Director of Rehabilitation.
Qualifications: Master’s Degree in Speech-Language Pathology required. Six (6) months as a Speech Pathologist in a long term care facility required. N.Y.S. Certification in Clinical Competence in Speech Language Pathology required. BLS certification required. One (1) year long term care/sub acute adult care preferred.
